29 CFR 1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: Majestic Plastics, Inc., located at 811 North Main Street in Bellefontaine, Ohio; On or about January 27, 2025, the employer did not establish and implement a written respiratory protection program with required worksite-specific procedures and elements outlining: (1) the selection of respirators for use, (2) medical evaluations of employees, (3) fit testing procedures, (4) cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators, (5) Training of employees in the respiratory hazards to which they are potentially exposed, (6) Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance, and (7) procedures for regularly evaluating the effectiveness of the program. Employees applying 3M Adhesion Promoter 4298UV4 in the assembly department were required to wear Project Source N95 Respirator Masks (5302682). Employees were exposed to hazards associated with cyclohexane, xylene, and ethylbenzene such as, but not limited to, dizziness, skin and eye irritation, cough, headaches, and respiratory tract irritation.
