December 23, 2024, a work-related injury occurred in a community home jobsite that resulted from an act of workplace violence where an employee experienced days away from work. b) Corporate Location, 12450 Perry Highway Wexford PA 15090, On or about January 23, 2025- The employer did not record the following workplace injury OSHA Form 300 or equivalent for the calendar year 2025. January 19, 2025, a work-related injury occurred in a community home jobsite that resulted from an act of workplace violence where an employee experienced medical treatment from an hospital emergency room.
