29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) Big Comet Rotary Thermoformer - The Big Comet Rotary Thermoformer had electrical, gas, pneumatic, and thermal energy. The employer had a machine-specific lockout/tagout procedure for this machine; however, this procedure only included electrical energy but did not include gas, compressed air, and residual heat as energy sources. The machine-specific procedure did not include steps for how to de-energize and lockout/tagout the gas and compressed air nor did the procedure address the residual heat, on or about January 21, 2025. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
