29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Employee(s) using a 100-foot red extension cord to power a tile saw on the 1st floor, north end of building located at 16950 W. 86th Street, Lenexa, Kansas, were exposed to electrical hazards due to outer jacket of the extension cord pulling away from the plug end of the cord, thus exposing energized conductors.
