29 CFR 1910.140(c)(13)(i): Anchorages were not capable of supporting at least 5000 pounds (22.2 kN) for each employee attached. Location - 55 Littleton Road, (behind Apt 14), Ayer, MA. On or about January 29, 2025, an employee, while working at a height greater than twenty feet, fell to a lower level when the branch to which the lifeline was attached could not hold their weight and equipment.
