29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Roof, 25695 Gray Stone Drive, Madison, AL: On or about January 29, 2025, the employer exposed employees to fall hazards, in that the employer did not ensure that the employees were wearing fall protection or installed guard rails around the unprotected sides and edges of the 10/12 pitch residential roof while performing shingle installation activities at a height of approximately 9 feet, 1 inch to the ground below.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Roof, 25695 Gray Stone Drive, Madison, AL: On or about January 29, 2025, the employer exposed employees to fall hazards,  in that the employees did not recognize fall hazards while engaged in shingle installation activities on the 10/12 pitch residential roof with unguarded edges/sides at a height of approximately 9 feet, 1 inch to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Roof, 25695 Gray Stone Drive, Madison, AL: On or about January 29, 2025, the employer exposed employees to fall hazards, in that the employees used an aluminum ladder to gain access and egress to the rear 10/12 pitch roof eave that did not extend at least 3 feet above the upper landing at a height of approximately 9 feet, 6 inches to the ground below while employees were engaged in roof shingle installation activities.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  (a) Roof, 25695 Gray Stone Drive, Madison, AL: On or about January 29, 2025, the employer exposed employees to fall hazards, in that the employees engaged in roof shingle installation activities used an aluminum ladder to gain access and egress to the rear 10/12 pitch roof eave were not trained in ladder hazard recognition.
