1926.600(a)(6)(iv) A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means: Location: 116 Old Main Road South Yarmouth MA On or about January 30, 2025, employees were exposed to electrical shock hazards while working where no designated spotter was identified to assist the operator in maintaining clearance from energized power lines.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about January 28, 2025, employee was admitted to Brigham and Women's Hospital burn clinic for treatment of electric shock and subsequent electrical burns and was not reported by the employer.
