29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, caustic liquids, or chemical gases or vapors and at times prior thereto, workers were exposed to a struck by hazard while cutting concrete with a saw, without the use and wear of protective eyewear: a. At a residential construction north side located at 823 Cordova Palms St. Agustine FL. On or about January 30, 2025, the employer exposed employees to struck-by hazards, in that protective eyewear was not utilized by employees while using impact drivers for solar panel installation.

On or about January 30, 2025, the employer was exposing 3 employees to a 9 1/2-foot fall hazard, in that the employees were not wearing fall protection when installing solar panels.
