29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On or about January 30, 2025, at the above addressed jobsite, during the positioning of trusses from a crane an employee was not wearing a hard hat to protect against falling materials and debris and struck by hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   a. On or about January 30, 2025, at the above addressed jobsite, employees performing commercial carpentry operations from a JLG 460SJ aerial lift, approximately seventeen (17) feet above the ground, were not wearing a personal fall arrest system, thereby exposing the employees to fall hazards.

On or about January 30, 2025, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
