29 CFR 1926.451(e)(2)(iii): When hook-on and attachable ladders were used on a supported scaffold more than 35 feet (10.7 m) high, they did not have rest platforms at 35-foot (10.7 m) maximum vertical intervals: a. On or about January 27, 2025, at 10059 Carter Road PALMETTO, FL 34221, elevator tower/shaft #3, employees were performing masonry work from a supported scaffold, wherein the hook-on and attachable ladders that were used on the supported scaffold which was more than 35 feet (10.7 m) high, did not have rest platforms at 35-foot (10.7 m) maximum vertical intervals; therefore exposing the employees to a fall hazard.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a. On or about January 27, 2025, at 10059 Carter Road PALMETTO, FL 34221, elevator tower/shaft #3, employees were performing masonry work from a supported scaffold, wherein the work platform/tier was more than 10 feet above the lower level, and during which time the employee was not protected from falling by a guardrail system nor the use of personal fall arrest systems; therefore exposing the employees to a fall hazard.
