This condition exposed employees to cuts and struck-by hazards. (b)TrueNorth Steel at 220 4th Street, Huron, SD 57350: On or about and at times prior to January 29, 2025, the employer did not ensure the Spiral Pipe Mill 1 Dump Table was properly guarded to prevent material roll off. This condition exposed employees to struck-by hazards.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields: (a)TrueNorth Steel at 220 4th Street, Huron, SD 57350: On or about and at times prior to February 6, 2025, the employer did not ensure employees in the Helical Mill 1 area were properly protected from arc welding rays. This condition exposed employees to ultra-violet radiation hazards associated with direct line of sight unguarded welding activities.
