29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: Employees were exposed to hazards and injury resulting from unexpected energization of machines during maintenance, in that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At a work site located at 6401 E. 40th Street, Kansas City, Missouri. Employees were exposed to an amputation hazard when using the disk sander serial number GG840. employees were exposed to point of operation or other serious injuries from the machine, in that the sander was not covered or guarded to prevent rotating parts or flying chips/sparks from contacting the operator.
