29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: Establishment - An employee struck a piece of warehouse racking causing serious damage to the racking while operating a Powered Industrial Truck. The operator was allowed to operate Powered Industrial Trucks without receiving refresher training.
