29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about Feb 4, 2025, employees of S&A Equipment & Builders LLC were performing sewer construction work near 116 North Street, Hannibal, Missouri. Employees were grading rock to installing a section of sewer and repairing a pipe while inside the trench measuring approximately 18 feet wide x over 20 feet long x 11.9 feet deep. The trench did not have a protective system designed in accordance with paragraph (b) or (c). Employees were exposed to engulfment and struck-by hazards.

Employees were grading rock to installing a section of sewer and repairing a pipe while inside the trench measuring approximately 18 feet wide x over 20 feet long x 11.9 feet deep. The road next to the trench had been undermined and there was no support system used to support it. Employees working in the trench were exposed to engulfment and struck-by hazards.

The spoils pile was not located at least two feet from the edge of the excavation. The weight of the spoils on the trench wall exposed employees to engulfment and struck-by hazards.
