Employees were grading rock to installing a section of sewer and repairing a pipe while inside the trench measuring approximately 18 feet wide x over 20 feet long x 11.9 feet deep. The trench did not have a protective system designed in accordance with paragraph (b) or (c). Employees were exposed to engulfment and struck-by hazards.
