At the jobsite located at 5354 Birch Street, Roeland Park, Kansas, employees using a portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 9 feet from the roof to the ground. The side rails of the portable extension ladder did not extend at least 3 feet above the landing surface of the roof. The side rails extended approximately 18 inches.
