29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about February 5, 2025 the employer did not ensure that an annual inspection of the procedures for the control of hazardous energies on equipment had been performed. Equipment within the facility in which employees service and maintain include but are not limited to presses, mechanical power presses, shears, press brakes, and printing presses. Hazardous energies present on equipment include but are not limited to hydraulic, mechanical, electrical, and pneumatic.

29 CFR  1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  On or about February 5, 2025, the employer did not ensure that employees had received training as authorized for the control of hazardous energies. Employees are tasked with the service and maintenance of equipment including setup and change overs which require the control of hazardous energies. Equipment utilized in the facility includes but is not limited to presses, mechanical power presses, press brakes, shears, and printing presses. Hazardous energies present include but not limited to hydraulic, pneumatic, electrical, and mechanical.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about February 5, 2025, the employer did not ensure that effective machine guarding was in place to protect employees from the hazardous area on the backside of shears (3) at the facility. Employees are exposed to the pinch point created by the downward motion of the blade as well as the pinch point created by the conveyor belt located in the same area. A hinged barrier guard is in place; however, the guard is not interlocked, nor does it completely cover the hazardous area.  b)  On or about February 5, 2025, the employer did not ensure that effective machine guarding was in place to protect employees from the hazardous areas of the Mark Andy 830 printing press. Guarding was not in place to protect employees from the moving machine gears, pulleys, and rollers. Employees are tasked with the operation of the equipment which includes making adjustments and changes during operation.

29 CFR  1910.217(c)(1)(i):The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es);   On or about February 5, 2025 the employer did not ensure that effective machine guarding was in place to protect employees from the point of operation on the Rouselle 40 ton mechanical power press. The opening in the present guard contained too large of an opening allowing employees to reach the point of operation. The opening was not within the guidelines of table O-10 of this standard.
