29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Spring Creek Corp., on or about February 4, 2025, at the location of 15110 Hardin Wapak Rd. Anna, OH: The employer did not ensure that employees had been adequatly trained in the recognition and avoidance of hazards specific to the operation of a 2015 Terex Front Discharge Mixer. Operating employees performing cleanout task without the benefit of this training were exposed to caught in hazards associated with drum rotation.
