On or about February 6, 2025, the employer failed to ensure hazardous energy control procedures were developed, documented, and/or utilized for the control of the potentially hazardous energy when employees were engaged in service and maintenance activities, such as cleaning and setting up machines and equipment, including but not limited to, the Webster Vertical Turning Machine, VTL #1, Bullard Vertical Turning Lathe, Buffalo Forge Co., Iron Worker Machine.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or about February 6, 2025, the employer failed to ensure machine guarding was adequately provided to protect operators from machine hazards created by the point of operation and rotating parts of the Webster Vertical Turning Lathe, known as the "VTL #1". b. On or about February 6, 2025, the employer failed to ensure machine guarding was adequately provided to protect operators and other employees in the machine area from machine hazards created by the ingoing nip points and rotating parts of the discharge end of the inclined scrap conveyor of the Giddings and Lewis CNC Milling Machine. c. On or about February 6, 2025, the employer failed to ensure machine guarding was adequately provided to protect operators from the machine hazards created by the point of operation of the punch press portion of the Buffalo Forge Co., Iron Worker Machine. d. On or about February 6, 2025, the employer failed to ensure machine guarding was adequately provided to protect operators from the rotating parts of the non-working portion of the blade between the guide rollers and the band saw wheel housing of the HEM Saw, Model H90A-1 Horizontal Band Saw.
