Location: Worksite; Basement of building 2221. a) On or about 01/13/2025, the employer did not report to OSHA within 24 hours upon learning about the in-patient hospitalization of an employee who suffered a broken leg after falling 2-3 feet off of a A-frame ladder. The incident was reported to OSHA on 02/05/2025.
