The hazardous condition was not corrected before operation.  Employees were exposed to struck-by hazards while using the crane to hoist steel materials/parts used in the manufacturing/assembly process, weighing approximately 3,743 pounds.

Employees were exposed to struck-by and electrical shock hazards while using a crane  with damaged electrical connections to hoist steel materials/parts used in the manufacturing/assembly process.  b.  Gradall Industries, Inc located at 406 Mill Avenue SW,  New Philadelphia, OH 44663: On or about February 6, 2025, and at times prior, the employer did not examine and identify electrical defects such as but not limited to damaged flexible power cord/strain relief  associated with the  2 ton underhung crane  (EQ #30670, Crane # 79, Serial # 110897) with a Anver Vacuum and Walker Electric Magnetic Lifting Devices . Employees were exposed to struck-by and electrical shock hazards while using a crane with damaged electrical connections to hoist steel materials/parts used in the manufacturing/assembly process.
