29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):   (a)GR Truss & Components, LLC at 36629 258th Street, Kimball, SD 57355: On or about and at times prior to March 27, 2025, the employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative, including but not limited to the following:   (1) HP Component Saw (2) Industrial Electric Compressors   This condition exposes authorized and affected employees to shock / electrocution hazards.  Abatement Note: As part of an energy-control program, employers must:  (1) Establish energy-control procedures for removing the energy supply from machines and for putting appropriate lockout or tagout devices on the energy-isolating devices to prevent unexpected reenergization. When appropriate, the procedure also must address stored or potentially reaccumulated energy.   (2) Train employees on the energy-control program, including the safe application, use, and removal of energy controls; and   (3) Inspect these procedures periodically (at least annually) to ensure that they are being followed and that they remain effective in preventing employee exposure to hazardous energy.   The energy-control procedures must outline the scope, purpose, authorization, rules, and techniques that employees will use to control hazardous energy sources, as well as the means that will be used to enforce compliance. These procedures must provide employees at least the following information:   (1) A statement on how to use the procedures;  (2) Specific procedural steps to shut down, isolate, block, and secure machines;   (3) Specific steps designating the safe placement, removal, and transfer of lockout/tagout devices and identifying who has responsibility for the lockout/tagout devices; and   (4) Specific requirements for testing machines to determine and verify the effectiveness of lockout devices, tagout devices, and other energy-control measures.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   (a)G R Truss & Components, LLC at 36629 258th Street, Kimball, SD 57355: On or about and at times prior to March 27, 2025, the employer did not ensure employees were properly trained on the use and operation of the Doosan 35 LPG forklift prior to moving trusses and other components/projects within the warehouse area and when transitioning in between the warehouse and the yard. This condition exposed employees to struck-by hazards associated with improper forklift operations.  (b)G R Truss & Components, LLC at 36629 258th Street, Kimball, SD 57355: On or about and at times prior to March 27, 2025,  the employer did not ensure employees were properly trained on the use and operation of the Manitou M-40 diesel forklift prior to moving trusses and other components/projects within the warehouse area and when  transitioning in between the warehouse and the yard. This condition exposed employees to struck-by hazards associated with improper forklift operations.  (c)G R Truss & Components, LLC at 36629 258th Street, Kimball, SD 57355: On or about and at times prior to March 27, 2025,  the employer did not ensure employees were properly trained on the use and operation of the Komatsu propane forklift prior to moving trusses and other components/projects within the warehouse area and when transitioning in between the warehouse and the yard. This condition exposed employees to struck-by hazards associated with improper forklift operations.  Abatement Note: Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

29 CFR  1910.213(r)(3): For combination or universal woodworking machines each point of operation of any tool shall be guarded as required for such a tool in a separate machine:   (a)GR Truss & Components, LLC at 36629 258th Street, Kimball, SD 57355: On or about and at times prior to March 27, 2025, the employer did not ensure employees were protected from injury in that, the saw on the HP Component Saw Machine was not effectively guarded.  This condition exposed employees to caught-in and amputation hazards.

29 CFR  1910.242(b): Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment:   (a)G R Truss & Components, LLC at 36629 258th Street, Kimball, SD 57355: On or about and at times prior to March 27, 2025, the employer did not ensure that compressed air used for cleaning was reduced to less than 30 p.s.i.  Pneumatic air nozzles were identified throughout the plant which are used to clean off the equipment and personnel that did not have pressure regulation or chip guarding controls in place. The air nozzle had pressure in excess of 90 p.s.i. This condition exposed employees to struck-by hazards and pulmonary embolism.

29 CFR  1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a)G R Truss & Components, LLC at 36629 258th Street, Kimball, SD 57355: On or about and at times prior to March 27, 2025, the employer did not ensure that a written hazard communication program was develop and implemented, which at least described how the criteria specified for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also included a hazardous chemicals list and methods used to inform employees of the hazards of non-routine tasks, had not been developed for employee exposures, to include,  but not limited to, the following: chemicals:      1) Motor Oil 2) Anti-freeze 3) Hydraulic Oil 4) Spray Paint 5) Fungicide (not labeled) 6) Industrial Cleaning Solutions (not labeled)    Abatement Note:  The written hazard communication program will also include:   (1) A list of the hazardous chemicals known to be present using an identity that is referenced on the appropriate safety data sheet (the list may be compiled for the workplace as a whole or for individual work areas) (2) The methods the employer will use to inform employees of the hazards of non-routine tasks.  (3) The location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets required by this section.
