29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles. At a residential worksite located at 800 Galveston Dr, Liberty MO -- employees engaged in tearing off a pre-existing roof using pitchforks, hammers and other various small hand tools were not wearing any eye protection. This exposed the employees to getting shrapnel and/or debris in their eyes causing injury. This was documented on or about 2/6/2025.

This exposed the employees to fall hazards ranging from approximately 8' to excess of 17' to ground level. This was documented on or about 2/6/2025.
