29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. Location: Worksite; right side of house. a) On or about 02/07/2025, an employee using a 40 foot extension ladder to access the roof was exposed to fall hazard of up to approximately 25 feet to the ground below from the ladder not extending at least 3 feet past the upper landing surface.
