(a) South electrical room attic access; On or about January 31, 2025, while accessing the attic using a 10-foot Louisville stepladder, employees were exposed to falls due to the side rails not extending 3 feet above the attic landing surface or the top of the ladder not being secured to prevent deflection while mounting and dismounting the ladder.
