29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) Primary Production, Line 23 - On or about January 30, 2025, employees were exposed to machine hazards associated with pinch-points and rotating parts while lubricating the chain on an infeed conveyor. An employee's right hand was caught in the chain and sprocket, causing fingertip amputations of the ring, middle, and index fingers.

(b)Primary Production, Line 23: On or about January 30, 2025, the employer failed to ascertain the exposure status of the employee tasked with lubricating the chain on an infeed conveyor which supposed to be done when the conveyor?s cord was unplugged and within the exclusive control of the employee responsible for lubricating, exposing said employee exposed to machine hazards associated with pinch-points and rotating parts
