29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a)Dakota Redi-Mix, Inc. at Silver Street & 14th Street Centennial Park, Sturgis, SD 57785: On or about and at times prior to February 7, 2025, the employer did not ensure that employees conducting underground sanitary sewer line installation within an unprotected excavation greater than five feet in depth were protected from trench wall cave-in by an adequate protective system. This condition exposed employees to struck-by and cave-in hazards associated with a trench wall collapse.
