A) 1050 North Broadway - Yonkers - NY: Two employees were working in an aerial lift that was going up to approximately 30-feet high. The employees were not tied of by harnesses, restraints, or by any other fall protection devices while working in the lift, on or about, November 14th, 2024.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. A) 1050 North Broadway - Yonkers - NY: Employees were exposed to fall hazards of approximately 60-feet while engaged in carpentry work on the top of a building. The employees were not protected by any type of fall protective devices, or a fall protection system, on or about November 14th, 2024.
