29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: A) 65 Forrest Avenue, Rye - NY: Employees were exposed to fall hazards of approximately 14-feet while using an extension ladder to access a roof. The extension ladder that was used, was only extended approximately 1 1/2-feet over the landing, on or about, December 13th, 2024.
