On or about February 5, 2025, and at times prior thereto, the employer failed to provide refresher training at least every three years to each employee involved in operating a process to assure the employee understands and adheres to the current operating procedures of the covered process.

29 CFR 1910.119(j)(2): The employer shall establish and implement written procedures to maintain the on-going integrity of process equipment On or about February 5, 2025, and at timer prior thereto, in the ammonia refrigeration engine room the employer failed to ensure detailed written procedures were established and implemented for process equipment such as but not limited to: a) Compressor safety cutouts, b) Corrosion Under Insulation (CUI) for ammonia vessels and piping.
