29 CFR 1910.217(b)(7)(iii): Fixing of the selection was not by means capable of supervision by the employer. On or about February 10, 2025, the employer did not ensure that the means (keys) of selecting Off, "Inch," Single Stroke, and Continuous (when the continuous function is furnished) were in control of the supervisors. Keys for selection of modes on mechanical power presses was left in the controls capable of being accessed by operators during operation.

29 CFR 1910.217(c)(1)(i):The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es): On or about February 10, 2025 the employer did not ensure that effective guarding was in place on the Minster p2-100-48 (press 5) mechanical power press. Employees operated the press in fully automatic mode, at which time the barrier guarding on the exit portion of the press did not prevent the employee from reaching the point of operation. Openings in the guarding did not meet the specifications of table O-10 of the standards.

29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation; On or about February 10, 2025 the employer did not ensure that employees tasked with service and maintenance of presses within in the facility had received training as authorized employees. Employees who operate the equipment perform tasks that require the isolation of hazardous energies which include setup, coil changes, and die changes. These energies present include but are not limited to pneumatic, mechanical, electrical, and gravitational. Atlantic Tool & Die Company was previously cited for a violation of this occupational safety and health standard, 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1652561 , citation number 1, item number 3 and was affirmed as a final order on 3/23/2023, with respect to a workplace located at 4995 Atlantic Drive, Seville, OH 44273.
