29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: (a) Establishment, Plant - On or about February 10, 2025, Employees working on the upper level of the plant were required to use the forklift ramp to access the area, as the designated pedestrian passageway was obstructed with stored product.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: (a) Establishment, Warehouse - On or about February 10, 2025, employees were operating a Tailift Model FG18 forklift with no working horn to alert employees working in the area. (b) Establishment, Warehouse - On or about February 10, 2025, employees were operating two forklifts with no safety mirrors to aid the employees in the safe operation.
