Positioned on the side of a residential structure located at 3191 Garden Brooks Road, Jacksonville, FL: On or about February 10, 2025, the employer exposed employees to fall hazards, in that a 16-foot Werner extension ladder was used to access the top of the structure and did not extend three feet above the eave as required.
