Location: Job site; On or about 02/10/2025, employees were allowed to perform chimney repair work from a residential roof that was 20-feet above the ground below without the use of any form of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder. Location: Job site; On or about 02/10/2025, employees utilized a Little Giant and Werner portable extension ladder to access the upper roof area to perform chimney restoration work and the side rails of both ladders did not extend at least 3-feet above the roof which they were being used to gain access to.
