OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees as employees were exposed to struck-by hazards:  a.)  1351 Eisenhower Boulevard, Harrisburg, PA, garage/shop area, on or about February 11, 2025, employees were exposed to struck-by hazards from the storage racking system located outside the chemical storage room as the racking system was not anchored to the supporting surface.  b.)  1351 Eisenhower Boulevard, Harrisburg, PA, garage/shop area, on or about February 11, 2025, employees were exposed to struck-by hazards from the storage racking system located outside the chemical storage room that did not have the load limit labeled on the racking system.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.157(c)(1):  Portable fire extinguishers were not mounted, located, and identified so that they were readily accessible without subjecting the employees to injuries:  a.)  1351 Eisenhower Boulevard, Harrisburg, PA, garage/shop area, on or about February 11, 2025, a Kidde multi-purpose dry chemical fire extinguisher (S/N:  G44982821) was mounted to the wall with a fork-style mounting bracket that was coming out of the drywall.   b.)  1351 Eisenhower Boulevard, Harrisburg, PA, classroom area, on or about February 11, 2025, an Amerex B456 multi-purpose dry chemical fire extinguisher (S/N:  SH-865802) was located on the floor in the egress route from the classroom, and was not mounted.  IN THE ALTERNATIVE:  29 CFR 1910.37(a)(3):  Materials and/or equipment were placed, either permanently or temporarily, within the exit route:  a.)  1351 Eisenhower Boulevard, Harrisburg, PA, classroom area, on or about February 11, 2025, an Amerex B456 multi-purpose dry chemical fire extinguisher (S/N:  SH-865802) was located on the floor in the egress route from the classroom.  Abatement certification required within 10 days after abatement date. 29 CFR 1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a.)  1351 Eisenhower Boulevard, Harrisburg, PA, garage/shop area, on or about February 11, 2025, a yellow flexible electrical extension cord was running to an orange flexible electrical extension cord, which then powered a relocatable power tap and a Milwaukee charger.  Plugging one extension cord into another extension cord is prohibited by the flexible electrical extension cord's listing and labeling.  b.)  1351 Eisenhower Boulevard, Harrisburg, PA, garage/shop area, on or about February 11, 2025, a yellow flexible electrical extension cord was running to an orange flexible electrical extension cord, which then powered a relocatable power tap and a Milwaukee charger.  Relocatable power taps are intended to be directly connected to a permanently installed branch-circuit receptacle outlet, and are not intended to be series connected (daisy chained) to extension cords.  c.)  1351 Eisenhower Boulevard, Harrisburg, PA, garage/shop area, on or about February 11, 2025, a yellow flexible electrical extension cord was running to an orange flexible electrical extension cord, which then powered a relocatable power tap and a Milwaukee charger.  The relocatable power tap was cable tied to a structural support column.  Relocatable power taps are not intended to be permanently secured to building structures or similar structures.  d.)  1351 Eisenhower Boulevard, Harrisburg, PA, garage/shop area, on or about February 11, 2025, a yellow flexible electrical extension cord was running to an orange flexible electrical extension cord, which then powered a relocatable power tap and a Milwaukee charger.  The relocatable power tap was cable tied to a structural support column.  Relocatable power taps are not intended to be used as a substitute for fixed wiring.  Abatement certification required within 10 days after abatement date. 29 CFR 1910.305(g)(1)(iv):  Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv):  a.)  1351 Eisenhower Boulevard, Harrisburg, PA, garage/shop area, on or about February 11, 2025, a yellow flexible electrical extension cord was running to an orange flexible electrical extension cord, which then powered a relocatable power tap and a Milwaukee charger.  The use of the flexible electrical extension cords were prohibited in that the flexible electrical extension cords were being used as a substitute for the fixed wiring of the structure.  b.)  1351 Eisenhower Boulevard, Harrisburg, PA, garage/shop area, on or about February 11, 2025, a yellow flexible electrical extension cord was running to an orange flexible electrical extension cord, which then powered a relocatable power tap and a Milwaukee charger.  The use of the orange flexible electrical extension cord was prohibited in that the orange flexible electrical extension cord was attached to a structural column with a cable tie and duct tape.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
