OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that a dust collector containing explosive combustible dust (Kst = 3.45) was located adjacent to a chemical drying oven within a laboratory setting, exposing employees to potential deflagration, explosion, or fire:  (a) Polymer Drying & Mixing Room P132  -  On or about February 18, 2025, the employer did not ensure the dust collector, connected via ductwork to a Blue M Class A Batch Oven was installed outdoors or with proper separation from ignition sources and consideration for the installation of engineered explosion protection systems.  Pursuant to 29 CFR 1903.19, the employer must submit abatement certification and documentation, required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.106(e)(6)(ii):  Grounding. Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100�F (37.8�C), were dispensed into containers without the nozzle and container being electrically interconnected as required:  (a) Chemical Storage Room P143 - On or about February 11, 2025, the employer did not ensure that three (3) 55-gallon drums of isopropyl alcohol and one (1) 55-gallon drum of methanol, a total of four (4) containers of Category 2 flammable liquid chemicals having flashpoints of 55 �F and 52�F, respectively, were electrically interconnected for dispensing, exposing employees to the potential for fire or explosion.  (b) Polymer Drying & Mixing Room P132  -  On or about February 11, 2025, the employer did not ensure 55-gallon drums containing a slurry of polymer and ethanol, a Category 2 flammable liquid having a flashpoint of 57 �F, were grounded while in storage and electrically interconnected to a drum roller as these drums were rolled to mix the slurry, exposing employees to the potential for fire or explosion.  Pursuant to 29 CFR 1903.19, the employer must submit abatement certification and documentation, required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.212(a)(4): Revolving drum(s), barrel(s), or container(s) were not guarded by enclosure(s) which were interlocked with the drive mechanism so that the barrel(s), drum(s), or container(s) could not revolve unless the enclosure(s) were in place:  (a) Polymer Drying Room:  On or about February 18, 2025, the employer failed to guard a revolving 55-gallon drum filled with a slurry of polymer and ethanol exposing employees to an entanglement hazard.
