29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part. On or about February 11, 2025, the employer did not have a program in place to identify hazards found on the job site, 701 N Main St in Clyde, Ohio 43410. This program assists employees in maintaining a safe working environment and supports the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls from at least sixteen feet and the use of personal protective equipment required for job tasks.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about February 11, 2025, the employer, working at the commercial address of 701 N Main St in Clyde, Ohio 43410, did not ensure their employees had been properly trained on how to use appropriate fall protection systems including but not limited to the personal fall arrest system employers had onsite or alternative fall protection measures. Employees were removing parts of an old roof and preparing to secure new metal roofing material to the roof of a multi-story commercial building, or armory, without the use of fall protection, exposing employees to falls hazards of over 16 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about February 11, 2025, the employer, working at the commercial site address of 701 N Main St in Clyde, Ohio 43410, did not ensure their employees were protected from fall hazards by properly using their personal fall arrest system. Employees were removing parts of an old roof and securing new underlayment roofing material to the 7/12 pitch roof of a multi-story residential building without properly being anchored in or tied off to their fall protection, exposing employees to fall hazards of over 16 feet.
