29 CFR  1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  Employees were exposed to electrical shock hazards in that the electrical box covers for all electrical boxes were missing, exposing energized wires. There was no protection from contacting the exposed energized wires inside the electrical boxes in the following instances throughout the crematory storage and adjacent office areas:  1) Light switch in main storage room.  2) Light switch in occupied office. 3) Light switch in hallway. 4) Electrical outlet in occupied office.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  Employees exposed to health hazards, in that chemical portable pressure spray bottles were unlabeled. A chemical spray bottle containing Prochem All Fiber Rinse carpet cleaner, and a chemical spray bottle containing Roundup were not labeled located in the basement storage room.
