29 CFR 1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: At the worksite located at 7475 West US Highway 60 Republic, MO: Employees were exposed to respiratory hazards. Employees use respirators such as N95s and airlines. The use of N95s is voluntary and the use of airlines is mandatory. The employer did not implement and establish a written respiratory program.

29 CFR 1910.253(b)(4)(i):Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire; At the workplace located at 7475 West US Highway 60 Republic MO: Employees were exposed to fire and explosion hazards. Oxygen cylinders in storage were not separated from fuel-gas cylinders with a minimum distance of 20 feet or by a noncombustible barrier in several instances, including but not limited to: a.) A gas cylinder of oxygen was stored next to a gas tank of acetylene on the torch cart without a noncombustible barrier. b.) Acetylene cylinders were stored next to oxygen cylinders in the cylinder storage area without a fire barrier or twenty feet between.
