29 CFR 1910.217(c)(1)(i):The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es): a) On or about February 6, 2025, at the jobsite located at 795 West 20th Street Hialeah, FL 33010, an employee was exposed to a caught-by hazard while operating an Alva Allen mechanical power press (Model No. B2-101) that was not properly guarded at its point of operation and was being used to join metal corners. b) On or about February 6, 2025, at the jobsite located at 795 West 20th Street Hialeah, FL 33010, an employee was exposed to a caught by hazard while operating a mechanical power press that was not properly guarded at its point of operation and was being used to punch holes in metal. c) On or about February 6, 2025,, at the jobsite located at 795 West 20th Street Hialeah, FL 33010, an employee was exposed to a caught by hazard while operating an Alva Allen mechanical power press (model No. B2-101) that was not properly guarded at its point of operation and was being used to punch a hole in metal. d) On or about February 6, 2025, at the jobsite located at 795 West 20th Street Hialeah, FL 33010, an employee was exposed to a caught by hazard while operating an Alva Allen mechanical power press (model No. BT 5) that was not properly guarded at its point of operation and was being used to cut out grilles for air conditioning vents.
