On or about February 12, 2025, at the jobsite: an employee was exposed to 21'-5" fall hazards while working on a 3:12 pitched roof, in that, fall protection was not provided.   b. On or about February 12, 2025, at the jobsite: an employee was exposed to eye injury hazards while using a nail gun to install soffit and facia on a roof 3:12 pitched roof, in that, eye protection was not provided.
