29 CFR  1910.94(a)(5)(iv): For employees who use respirators required by this section, the employer must implement a respiratory protection program in accordance with 29 CFR 1910.134.  a) On or about and at times prior to March 26, 2025, at the establishment located at W3639 County Highway H, Chilton, Wisconsin; at the Chilton Saw Plant, the employer did not develop and implement a respiratory protection program where respirators were necessary to protect the health of each employee who used an abrasive blasting hood (RPB Nova 3) during abrasive blasting operations.  Each employee wearing an abrasive blasting hood was not medically evaluated prior to wearing that respirator exposing employees to inhalation hazards and inadequate physical/physiological capability, in-line air-purifying filters were not maintained and replaced or refurbished periodically following the manufacturer's instructions, and periodic carbon monoxide monitoring was not performed with a high-temperature alarm or carbon monoxide alarm was not installed on oil-lubricated compressors. 29 CFR  1910.134(c): This paragraph requires the employer to develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use. The program must be administered by a suitably trained program administrator. In addition, certain program elements may be required for voluntary use to prevent potential hazards associated with the use of the respirator.   a) On or about and at times prior to March 26, 2025, at the establishment located at W3639 County Highway H, Chilton, Wisconsin; at the Chilton Saw Plant, the employer did not develop and implement a respiratory protection program where respirators were necessary to protect the health of each employee who used an abrasive blasting hood (RPB Nova 3) during abrasive blasting operations. The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.  a) On or about and at times prior to March 26, 2025, at the establishment located at W3639 County Highway H, Chilton, Wisconsin; at the Chilton Saw Plant, the employer did not provide a medical evaluation to determine each employee's ability to use to a respirator, exposing employees to inadequate physical/physiological capability. Employees wore an abrasive blasting hood (RPB Nova 3) respirator that was necessary to protect the health of employees during abrasive blasting operations. Sorbent beds and filters shall be maintained and replaced or refurbished periodically following the manufacturer's instructions.  a) On or about and at times prior to March 26, 2025, at the establishment located at W3639 County Highway H, Chilton, Wisconsin; at the Chilton Saw Plant, the employer did not have suitable in-line air-purifying filters to ensure breathing air quality supplied to the abrasive blasting hood (RPB Nova 3), exposing employees to air contaminants. Prior to the inspection, the filter was last replaced on May 17, 2023. 29 CFR  1910.134(i)(7): For oil-lubricated compressors, the employer shall use a high-temperature or carbon monoxide alarm, or both, to monitor carbon monoxide levels. If only high-temperature alarms are used, the air supply shall be monitored at intervals sufficient to prevent carbon monoxide in the breathing air from exceeding 10 ppm.  a) On or about and at times prior to March 26, 2025, at the establishment located at W3639 County Highway H, Chilton, Wisconsin; at the Chilton Saw Plant, the employer did not have a carbon monoxide alarm or periodically monitor carbon monoxide levels to ensure breathing air quality supplied to the abrasive blasting hood (RPB Nova 3), exposing employees to air contaminants.
