29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent:  The Boldt Company- 2525 N Roemer Road, Appleton, WI.  On or about February 12, 2025, the OSHA Form 300 utilized by the employer to document work-related injuries and/or illnesses did not include column B information to identify the employee's name on the OSHA Form 300 or equivalent for calendar year 2025.  (a) On or about January 16, 2025, the name of an injured masonry employee (case No. 1) was not recorded on the employers calendar year 2025 OSHA Form 300 because the form was modified by removing column B.

29 CFR 1904.29(b)(1): The log of occupational injuries and illnesses, (OSHA Form No. 300), was not completed in the detail provided in the form and the instructions contained therein:  The Boldt Company- 2525 N Roemer Road, Appleton, WI.  On or about February 12, 2025, the employer did not provide the employee's name in column B, the jobsite location in column E, and/or provide a description to identify the object or substance that directly caused the injury in column F on the OSHA Form 300 or equivalent for calendar year 2025.  (a) On or about January 16, 2025, a masonry employee working on the roof of a building at a jobsite strained their left knee and lower back, which resulted in 19 days work transfer or restriction, case No. 1.  The injured employee's name was not identified in column B.  (b) On or about January 16, 2025, a masonry employee working on the roof of a building at a jobsite strained their left knee and lower back, which resulted in 19 days work transfer or restriction, case No. 1.  The specific jobsite where the injury occurred was not identified in column E.  (c) On or about January 16, 2025, a masonry employee working on the roof of a building at a jobsite strained their left knee and lower back, which resulted in 19 days work transfer or restriction, case No. 1.  The object or substance that directly injured the person was not described in column F.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  (a) Mayo Clinic Hospital, 2nd floor of Wish Art Building; On or about February 12, 2025, and dates prior, strain relief was not provided for a flexible cord end on an extension cord (flexible cord set), which was providing electrical power to a room air pressure monitoring system for their negative air-pressure enclosure.
