29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about February 6, 2025, and times prior to, employees of The Chest LLC, working at 2 Pauwels Drive in Washington, Missouri, were exposed to amputation and caught-in hazards when performing maintenance and servicing on equipment, including but not limited to the Automatan 4000 Series Litho Label Laminator. An annual periodic inspection of the energy control procedures was not performed by the employer to identify deficiencies, inadequacies, and employees responsibilities.

Such tools were not used in lieu of other guarding required by this section, but can be used to supplement protection provided On or about February 6, 2025, and times prior to, during the cleaning of the Automatan 4000 Series Litho Label Laminator at 2 Pauwels Drive in Washington, Missouri, the employer did not ensure hand tools such as a sponge were used when cleaning glue from a roller. An employee working in the unguarded danger zone was exposed to the machines rotating parts, point of operation while using a rag.
