At the worksite, located at 5725 New Tampa Highway Lakeland, FL 33815; On or about 2/13/25, the employer exposed employees to struck by and crushing hazards, in that secure and proper storage of boxed goods and materials were not utilized and provided for employees while performing warehouse shipping and receiving operations around defective & broken warehouse racking systems and with some areas of the storage racks not anchored to the floor.

At the worksite, located at 5725 New Tampa Highway Lakeland, FL 33815; On or about 2/13/25, the employer exposed employees to fire hazards, in that the rear emergency exit door was obstructed by a barrel &  industrial fans which exposes employees to fire and entrapment hazards.

At the worksite, located at 5725 New Tampa Highway Lakeland, FL 33815; On or about 2/13/25, the employer exposed employees to struck by hazards; in that, forklift operations were conducted without formal instruction, hands on trainings, evaluations and certifications in place while operating a Toyota electrical forklift moving merchandise and products in the front and back lots of the warehouse.

29 CFR  1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a.) At the worksite, located at 5725 New Tampa Highway Lakeland, FL 33815; On or about 2/13/25, the employer exposed employees to fire and electric shock hazards, in that extension cords were used were used as a permanent method and daisy chained; connected to multiple extension cords, power strips and to an outlet across the walking and working floor surface while energizing computer monitors, printer label machines, scanners and fans in the production areas of the facility.
