29 CFR  1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:  (a) On or about February 13, 2025, racking systems used for storage of parts and materials, created a crushing and struck-by hazard, in that the system was damaged in excess of the allowable limit of 0.5 inches of deflection within a 10 foot span, per ANSI MH16.1: 2021 'Specification for the Design, Testing and Utilization of Industrial Steel Storage Racks.'  Specifically, but not limited to the following damaged rack areas were identified: The racking system west of the dust collector had deflected/bent vertical members and bent cross sections.   (b) On or about February 13, 2025, racking systems used for storage of parts and materials created a crushing and struck-by hazard, in that the system was not bolted to the ground. The racking systems on the west side and the north side of the manufacturing area were not secured to the ground.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  (a) On or about February 13, 2025, the Cincinnati Lathe (model: 15) was not guarded at the rotating chuck, exposing employees to a caught-on hazard.  (b) On or about February 13, 2025, the Compass lathe (model:20/1500, serial:1534) was not guarded at the rotating chuck, exposing employees to a caught-on hazard.  (c) On or about February 13, 2025, the Wolf Lathe (model:482006, Serial:1954) was not guarded at the rotating chuck, exposing employees to a caught-on hazard.

29 CFR  1910.303(b)(7)(iv):There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  (a) On or about February 13, 2025,  the employer did not ensure that electrical panels located west of the dust collector did not pose an electrical hazard in that the cover panels were not attached all the way to the housing, creating an opening on the bottom section of the panels. This exposed employees to electrical shock hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.  (a) On or about February 13, 2025, the employer had not provided employees working with hazardous chemicals such as flammable liquids and combustible particulate solids with effective information and training as outlined in 29 CFR 1910.1200(h)(2) through (h)(3).  Information and training was not provided for the following elements:  (i) The requirements of 29 CFR 1910.1200, (ii) Specific operations in their work area where combustible particulate solids (combustible dusts) are utilized, (iii) Methods and observations used to detect the presence or release of hazardous chemicals in the work area, (iv) The physical and health hazards associated with flammable liquids, (v) The physical and health hazards associated with combustible particulate solids (combustible dusts), (vi) The measures employees can take to protect themselves from identified physical and health hazards associated with flammable liquids and combustible particulate solids (combustible dusts) such as safety procedures, emergency procedures, safe work practices, and PPE, and (vii) Explanation of container labeling systems.

29 CFR  1910.303(g)(1):Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  (a) On or about February 13, 2025,  in the manufacturer area, south of the dust collector, material was being stored directly in front of circuit breaker control panel,  which did not allow access to the electrical disconnect if an emergency occurred with the equipment and electrical power had to be immediately removed.
