29 CFR 1910.178(i)(1): Concentration levels of carbon monoxide gas created by powered industrial truck operations exceeded the levels specified in 1910.1000: At the worksite located at 6930 North Fremont Street Ozark, MO: On or around February 10th, 2025, employees were exposed to chemical hazards in the form of carbon monoxide. Employees of the shotshell department and receiving departments became ill after exposure to a carbon monoxide source including but not limited to a forklift.

29 CFR 1910.1000(a)(2):Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50ppm: At the worksite located at 6930 North Fremont Street Ozark, MO: On or around February 10th, 2025, employees were exposed to chemical hazards in the form of carbon monoxide. An employee was hospitalized after being exposed to an 8 hour Time Weighted Average concentration of 395.6 parts per million of carbon monoxide, which is 7.91 times the OSHA Permissible Exposure Limit (PEL) of 50 parts per million. Possible sources of exposure include but are not limited to forklifts and a heat exchanger.
