29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) On or about February 13, and at times prior, an employee operating a Messer Titan Plasma Cutter was not protected from potentially injurious light radiation by appropriate eye protection. Eye exposure to UV rays from plasma cutting may result in photokeratitis and corneal flash burns.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles to protect them from the rays when noncombustible or flameproof screens or shields were not used:  (a) On or about February 26, and at times prior, employees using a belt sander and operating a hydraulic press were not protected from rays from MIG welding. Eye exposure to welding rays may result in photokeratitis and corneal flash burns.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about February 13, 2025, and at times prior, employees were exposed to amputation and struck by hazards while using an unguarded hydraulic press machine to bend pieces of steel.  Two handed controls or an enclosure or guard was not provided on the point of operation for the ingoing nip points of the press machine.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4):  (a) On or about February 13, 2025, and at times prior, a Dewalt angle grinder that was not equipped with safety guard was used with an abrasive wheel. This condition exposed employees to contact with flying debris, and laceration hazards.
