29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a) 213 West 3rd St, La Junta, CO 81050: On or about February 26, 2025, and at times prior, the employer had not implemented a written respiratory protection program where employees required to wear tight-fitting respirators when sanding, spraying paint, and applying truck bed liner coatings. This hazard exposed the employee to the lack of knowledge of proper respirator use, limitations and potential body stresses.

29 CFR  1910.134(i)(6): The employer did not ensure that carbon monoxide (CO) levels in the breathing air from non-oil lubricated compressors did not exceed 10 ppm:  (a) 213 West 3rd St, La Junta, CO 81050: On or about February 26, 2025, and at times prior, an employee wore a supplied air respirator where breathing air was supplied from a non-oil lubricated compressor. No carbon monoxide monitoring equipment was installed in the breathing air system or otherwise used to ensure carbon monoxide levels in air supplied from the compressor did not exceed 10 ppm.  Potential injuries associated with exposure to carbon monoxide include, but are not limited to, headache, dizziness, weakness, upset stomach, vomiting, chest pain, and confusion.

29 CFR 1910.1000(a)(1): Employee(s) were exposed to an airborne concentration of Methylene bisphenyl isocyanate (also known as 4,4-diphenylmethane diisocyanate; MDI) listed in Table Z-1 in excess of the ceiling concentration of 0.02 parts per million (ppm):  (a) 213 West 3rd St, La Junta, CO 81050: On or about February 26, 2025, and at times prior, an employee applying Rhino TuffGrip coating was exposed to two time-weighted 15-minute exposures of 0.13 parts per million (ppm) and 0.11 ppm of Methylene bisphenyl isocyanate (also known as 4,4-diphenylmethane diisocyanate; MDI), approximately 6.5 and 5.5 times the ceiling concentration of 0.02 ppm. Exposure to MDI may result in bronchitis, pulmonary edema, lung damage, and sensitization which may result in allergic respiratory reactions.

29 CFR  1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):  (a) 213 West 3rd St, La Junta, CO 81050: On or about February 26, 2025, and at times prior, engineering or administrative controls were not implemented to reduce employee exposure to Methylene bisphenyl isocyanate (also known as 4,4-diphenylmethane diisocyanate; MDI) while applying truck bed liner. An employee applying liner was exposed to a time-weighted 15-minute exposure of 0.13 parts per million (ppm) of MDI, approximately 6.5 times the permissible exposure limit of 0.02 ppm. Exposure to MDI may result in bronchitis, pulmonary edema, lung damage, and sensitization which may result in allergic respiratory reactions.

29 CFR  1910.107(b)(9): A clear space of not less than 3 feet on all sides was not kept free from storage or combustible construction:  (a) 213 West 3rd St, La Junta, CO 81050: On or about February 26, 2025, and at times prior, combustibles including cardboard, wooden work benches, and aerosol cans were stored within 3 feet of spray booths in the shop area.  This condition exposed employees fire hazards.
