29 CFR 1910.147(c)(1):Energy Control Program. The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) Production Area: On or about May 5, 2025 the employer failed to an establish energy control program, which would include procedures, training, and periodic inspections to protect employees from hazards associated with unexpected startup and energy release from equipment when performing servicing and maintenance activities. Employees were exposed to crushing and amputation hazards when performing set-up activities on the Komatsu 45 Ton Mechanical Press. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.217(c)(1)(i):The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power presses. a) On or about January 29, 2025, the employer did not ensure that the 45 Ton Komatsu mechanical power press' point of operation was adequately guarded to protect employees from crushing and amputation injuries, in that the light curtain distance was not properly adjusted to prevent access prior to the cycle stopping. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
