29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  a) On or about February 18, 2025, located at Waterstone Lane Cir. Palm Beach FL. 33480, the employer did not provide helmets to  employees to protect from head injuries while working in close proximity to a building that had other employees working on the roof.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On or about February 18, 2025, located at  Waterstone Cir. Palm Beach FL 33480, the employer did not ensure that each employee exposed to approximately 30-foot fall hazards while on a walking/working surface with unprotected sides or edges due to the lack of fall protection.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a) On or about February 18, 2025, located at  Waterstone Cir. Palm Beach FL 33480, the employer did not ensure that ladders being used by workers to access a roof approximately 30 feet above the lower level secured at their top to a rigid support.
